Bradley County Vital Records

The Vital Records Unit of the Tennessee Department of Health maintains and issues all vital records for the state. Vital records cost $15 per copy and an additional $5 for each expedited request. To obtain a certified copy of a Bradley County divorce record, complete and sign the divorce application form, attach a photocopy of a state-issued ID or any ID alternatives acceptable by the office, and deliver the application in person or via mail to:

For mail-in applications, include a check or money order for a fee of $15. For in-person requests, the office is open from 8:00 a.m. to 4:00 p.m. on Monday, Wednesday and Friday. On Tuesday and Thursday, the office opens by 8:00 a.m. and closes by 6:00 p.m. Card payment or cash is accepted for in-person applications. Card payment attracts an additional non-refundable processing fee of $4.
